Abstract: 

Background and Aim: Mycosis fungoides (MF) is the most common form of primary cutaneous lymphoma, resulting from the infiltration of malignant T cells into skin tissues. The disease has three distinct stages: patch, plaque, and tumor. In the patch and plaque stages, it can mimic the clinical features of benign dermatoses. However, two scoring systems facilitate diagnosis at these stages, which will be discussed in more detail in this study. Methods: In this cross-sectional study, all formalin-fixed and paraffin-embedded skin specimens highly susceptible to MF based on clinical examination at the patch and plaque stages were collected from April 2017 to August 2019. They were subjected to H&E and IHC staining tests and examined according to Guitart and Pimpinelli criteria. Results: Out of 78 samples, 76 had histological criteria for MF according to Guitart’, s criteria, 54 were immunologically significant according to Pimpinelli’, s criteria for MF, and 52 were classified as definitive MF according to both criteria. CD3 and CD4 markers were the most frequent markers, respectively. In contrast to previous studies, the CD7 marker was expressed at 10% or higher in 24 cases. In addition, 65 of 78 samples had a CD8 marker, and only 13 samples were CD8-. Conclusion: In the early stages of MF, a single scoring system does not have sufficient sensitivity for the diagnosis. The triad of the patient’, s clinical presentation and histological and immunohistochemical features play a key role in achieving the correct diagnosis.

Abstract: 

Introduction: Mycosis fungoides (MF), a type of cutaneous T-cell lymphoma, is a rare condition with a variety of treatment options. A frequently utilized method in the treatment of early-stage MF is phototherapy. Full body phototherapy can be associated with photoaging of the skin and an increased risk of developing skin cancer. Targeted phototherapy, with a 308-nm excimer laser, may be just as effective and of a lower risk. This makes it especially useful in the treatment of patients with dysplastic nevi (DN) or other conditions which can put them at a higher risk of developing skin cancer. There are currently limited data on the treatment of early-stage MF with an excimer laser. Case Report: This study reports on a 43-year-old female patient presented to the clinic with early-stage (Ia) MF. Given her history of DN, she wished to pursue targeted phototherapy with an excimer laser. Localized light treatment was performed via a 308-nm excimer laser, 3 times weekly, for a total of 17 treatments to affected lesions. Following excimer laser treatment, she had a clinical resolution of her patches. On the follow-up clinic visits, she maintained her excellent response 12 months out. Conclusion: Targeted phototherapy with a 308-nm excimer laser may be a safer and equally effective alternative to generalized phototherapy in the treatment of early-stage MF. This case report demonstrates its efficacy and advantages over traditional generalized phototherapy.

Abstract: 

Contact dermatitis is an inflammatory skin disease due to contact with different substances and caused by two mechanisms: allergy and irritation. Contact dermatitis is a common skin disease and the most common occupational dermatosis. Differentiating between allergic and irritant contact dermatitis is essential in the identification of etiologic factor(s) and hence, its control and prognosis. This differentiation can be done only by patch test. Unfortunately, this simple, safe and useful test is always neglected. We review the basics of patch test, the method of performance, reading and interpretation of its results, possible side effects and results of studies done with patch test in different countries.

Abstract: 

We present a modified examplar-based inpainting method in the framework of patch sparsity. In the examplar-based algorithms, the unknown blocks of target region are inpainted by the most similar blocks extracted from the source region, using the available information. Defining a priority term to decide the filling order of missing pixels ensures the connectivity of the object boundaries. In the exemplar-based patch sparsity approach, a sparse representation of missing pixels is considered to define a new priority term and the unknown pixels of the fill-front patch is inpainted by a sparse combination of the most similar patches. Here, we modify this representation of the priority term and take a measure to compute the similarities between fill-front and candidate patches. Also, a new definition is proposed for updating the confidence term to illustrate the amount of the reliable information surrounding pixels. Comparative reconstructed test images show the effectiveness of our proposed approach in providing high quality inpainted images.

Abstract: 

Background: Various environmental factors can induce or exaggerate hand eczema. Objective: Evaluation of patch test results in hand eczema patients. Patients and Methods: In this descriptive study 100 patients with hand eczema referred to Loghman Hakim & Booali Hospital in Tehran underwent patch testing with 23 allergens obtained from Hennal Company. Results: 240 positive reactions observed in 86 patients. The most common positive reactions were related to nickel sulfate (30 patients, 13.5%) and potassium dichromate (28 patients, 11.7%). In positive group 28 patients were male and 58 were female. In negative group 3 patients were male and 11 were female (P>0.05). The mean age was 31.7±11.9 years and 24.8± 9.2 years for positive group and negative group, respectively (P<0.05). In positive patch test group 49 patients had subacute and 37 patients had chronic eczema. In negative patch test 11 patients had subacute eczema and 3 patients had chronic eczema (P>0.05). Conclusion: Performing patch test, the allergens involved in hand eczema can be detected. Detecting type of allergens, patients will be able to avoid those materials which can lead to improvement of disease.

Abstract: 

Background: Intima, media, and adventitia are three layers of arteries. They have different structures and different mechanical properties. Damage to intima layer of arteries leads to an inflammatory response, which is usually the reason for atherosclerosis plaque formation. Atherosclerosis plaques mainly consist of smooth muscle cells and calcium. However, plaque geometry and mechanical properties change during time. Blood flow is the source of biomechanical stress to the plaques. Maximum stress that atherosclerosis plaque can burden before its rupture depends on fibrous cap thickness, lipid core, calcification, and artery stenosis. When atherosclerotic plaque ruptures, the blood would be in contact with coagulation factors. That is why plaque rupture is one of the main causes of fatality. Method: In this article, the coronary artery was modeled by ANSYS. First, fibrous cap thickness was increased from 40 μ m to 250 μ m by keeping other parameters constant. Then, the lipid pool percentage was incremented from 10% to 90% by keeping other parameters unchanged. Furthermore, for investigating the influence of calcium in plaque vulnerability, calcium was modeled in both agglomerated and microcalcium form. Results: It is proved that atherosclerosis plaque stress decreases exponentially as cap thickness increases. Larger lipid pool leads to more vulnerable plaques. In addition, the analysis showed maximum plaque stress usually increases in calcified plaque as compared with noncalcified plaque. Conclusion: The plaque stress is dependent on whether calcium is agglomerated near the lumen or far from it. However, in both cases, the deposition of more calcium in calcified plaque reduces maximum plaque stress.

Abstract: 

Purpose: To investigate the efficacy of "dermal patch graft" in surgical management of peyronie"s. Materials and Methods: Eighteen of peyronie"s disease cases, with a mean age of 49 and a history of penile curvature and painful erection were enrolled in this study. Diagnosis was made clinically by plaque palpation. All of them were in the chronic stage of disease with symptom duration of at least 6 months. We also evaluate their potency through Brief Sexual Function Inventory (BSFI) questionnaire before and after the operations, meanwhile the degree of penile curvature was measured with goniometry while artificial erection status was induced. Results: Mean penile curvature, before and after the operation (58 and 5 degrees respectively), showed significant improvement (p<0.001). The improvement of curvature was irrespective of the plaque size. All of our patients suffered from inability to intercourse due to significant penile curvature but after the procedure 11 of them (66.1%) could do so. Also the BSFI score improved significantly in this subgroup (p<0.05). The remaining 7 cases (39%) already suffered from erectile dysfunction despite of operation; however, the penile curvature improved significantly in them. Six of this later group had a plaque size greater than 4 cm 2 and BSFI score was not significantly improved. Conclusion: Dermal patch graft as a cost effective method in the management of peyronie"s disease significantly corrects the curvature irrespective of plaque size and curvature severity. We found that if the fibrous plaque is less than 4 cm 2 and the patients has no severe erectile dysfunction, this procedure will significantly improve his potency; however, if the patient suffers from a plaque sized greater than 4 cm2 and / or severe erectile dysfunction, to reach satisfactory, implantation of penile prosthesis or applying other methods of artificial erection in addition to dermal patch graft is suggested.

Abstract: 

Introduction: Decreasing pain elicited by injections in children results in patient comfort during dental procedures and produces a positive experience towards dental procedures. The aim of the present study was to compare the effect of Dentipatch and 20% benzocaine gel in decreasing injection pain in children referring to Isfahan Faculty of Dentistry.Materials and Methods: Simple sampling method was used in the present clinical trial to select fifty-four 9-12 year-old children (24 males and 30 females). The injection site was dried with air spray for 5 seconds; then Dentipatch was placed by a dentist on the palate for 12 minutes at a distance of 5-10 mm from the gingival margin. A total of 1.8 mL of 2% lidocaine was injected; 10 minutes after the injection the same procedure was repeated on the contra-lateral side with the use of benzocaine gel. Data was collected and analyzed with pained t-test and Wilcoxon test using CPS and VAS scales (a=0.05).Results: Means of CPS in non-anxious children in the benzocaine and Dentipatch groups were 6.4 and 5.8, respectively, with no statistically significant differences (p value=0.081). Means of VAS in non-anxious children in the benzocaine and Dentipatch groups were 39.2 and 32.5, respectively, with no statistically significant differences (p value=0.068).Conclusion: Under the limits of the present study, it was concluded that there is no difference between the two methods regarding children’s reaction. Further evaluations are recommended with different application times, especially with the new Dentipatech method.

Abstract: 

PROPOSE: PEYRONIE’S DISEASE IS ONE OF THE MOST COMMON CAUSES OF PENILE CURVATURE AND PAINFUL ERECTION DUE TO FORMATION OF FIBROUS plaqueS ON PENILE TUNICA ALBUGINA. THE GOAL OF THIS STUDY WAS TO INVESTIGATE THE BENEFICIAL EFFECTS OF “DERMAL patch GRAFT” IN SURGICAL MANAGEMENT OF THIS CONDITION, WITH SPECIAL CONSIDERATION ON POST-OPERATIVE ANATOMICAL AND FUNCTIONAL RESULTS. METHODS & MATERIALS: 18 CASES (MEAN AGE=49 YEAR) OF PEYRONIE’S DISEASE, WITH HISTORY OF PENILE CURVATURE AND PAINFUL ERECTION, DIAGNOSED CLINICALLY WITH plaque PALPATION, WERE SELECTED. ALL OF THEM WERE IN CHRONIC stage OF DISEASE WITH SYMPTOM DURATION OF AT LEAST 6 MONTHS. WE ALSO EVALUATE THEIR POTENCY THROUGH BSFI QUESTIONNAIRE BEFORE AND AFTER THE OPERATION, MEANWHILE DEGREE OF PENILE CURVATURE DETERMINED WITH GONIOMETRY IN ARTIFICIAL ERECTION STATUS. RESULTS: MEAN PENILE CURVATURE, BEFORE AND AFTER THE OPERATION (58 AND 5 DEGREE RESPECTIVELY), SHOWED SIGNIFICANT IMPROVEMENT (P<0.001). THE IMPROVEMENT OF CURVATURE WAS IRRESPECTIVE TO THE plaque SIZE. ALL OF OUR CASES SUFFERED FROM INABILITY TO INTERCOURSE DUE TO SIGNIFICANT PENILE CURVATURE BUT AFTER THE PROCEDURE 11 OF THEM (66.1%) CAN DO SO. ALSO THE BSFI-SCORE IMPROVED SIGNIFICANTLY IN THIS SUBGROUP (P<0.05). THE REST OF 7 CASES (39%) ALREADY SUFFERED FROM ERECTILE DYSFUNCTION DESPITE OF OPERATION, HOWEVER THE PENILE CURVATURE IMPROVED SIGNIFICANTLY IN THEM.6 OF THIS LATTER GROUP HAD plaque SIZE GREATER THAN 4 CM AND BSFI-SCORE WAS NOT SIGNIFICANTLY IMPROVED IN THIS SUBGROUP. DISCUSSION & CONCLUSION: DERMAL patch GRAFT AS A COST EFFECTIVE METHOD IN THE MANAGEMENT OF PEYRONIE’S DISEASE SIGNIFICANTLY WILL CORRECT THE CURVATURE IRRESPECTIVE OF plaque SIZE AND CURVATURE SEVERITY. WE FOUND THAT IF THE FIBROUS plaque IS LESS THAN 4 CM AND THE PATIENT HAS NO SEVERE ERECTILE DYSFUNCTION, THIS PROCEDURE WILL SIGNIFICANTLY IMPROVE HIS POTENCY, HOWEVER IF THE PATIENT SUFFERED FROM plaque SIZE GREATER THAN 4CM AND/OR SEVERE ERE TILE DYSFUNCTION, OBTAIN A SATISFACTORY ERECTION, IMPLANTATION OF PENILE PROSTHESIS OR OTHER METHODS OF ARTIFICIAL ERECTION IN ADDITION TO DERMAL patch GRAFT IS SUGGESTED.
